The Practice accepts patients who are living within the Practice Area for more than 3 months. New patients will be asked to fill out a brief questionnaire to give their Personal details on a NHS Registration form. It is helpful if the new patient can bring along their NHS card or other means of identification. The Practice can only register official details.

NEW PATIENT HEALTH CHECK

New patients are asked to make a 10 minute appointment with the Nurses to have a Routine Health Check, please bring any medication boxes or list of medication to this appointment, and a sample of urine (please ask at Reception for a bottle). If you wish you may print off the New Patient Registration Form and the New Patient Health Questionnaire and fill these in prior to your arrival to save you time on the day.

EXPRESSING A PREFERENCE

Under the new contract patients are registered with the practice as a whole. However, the patient may prefer to see a particular Practionioner for their routine care and prescribing of medication and the Practice records who they are normally seen by in their record. The Practice would expect that the patient would normally go to the same Doctor for routine and repeat problems. However, if ther are problems that need to be seen more urgently the Patient must consult with a Doctor that is available.
